For security reasons, the form must be on a page that begins with https. If this page doesn’t show https (which happens on some browsers) in the url, go to the end of the URL and hit return or replace the http with https.
This is the beginning of a much longer form that must be filled in. Once you click on the ‘Sign up’ button on the bottom, you will receive an email with a link to add/change information as you gather more information. If you know how to fill in more fields, please do so since many of the fields must be filled in before you can be placed on the schedule.
